About Us

Evans Rehabilitation & Wellness provides expert, skilled and caring outpatient rehabilitation services to all patients referred for care.

Physical therapists evaluate, identify and treat impairments, functional limitations and disabilities related to pain, injury, disease or aging. They also work in the areas of prevention, education and management of injuries or impairments.

We offer personalized care programs that combine the clinical skills with one-on-one treatment in the following areas: orthopedic/musculoskeletal disorders, neurological disorders, and balance disorders. We also offer industrial health and women's health programs.

In addition to physical therapy, we offer a personalized fitness program and 24 hour health and wellness facility. Our gym is for people who do not need physical therapy treatment, but who are interested in optimizing their fitness with supervision by an injury prevention specialist. The 24 hour workout facility is available by membership.

Rehabilitation & Wellness is a physical therapy clinic and 24 hour gym that is dedicated to helping you improve your quality of life. Our Mission is to provide expert, skilled and caring services to all patients and clients. Physical therapy patients receive individual evaluations and treatment from a licensed clinical professional. Our gym has a professional staff of personal trainers available Monday-Friday from 7 a.m. to 6 p.m. and offers secure 24 hour access for use during non-staffed hours.

New therapy patients are seen promptly – we can typically accommodate you within 48 hours of your physician’s referral.

Personalized treatment programs are developed after careful analysis to meet your individual needs.
